COVID-19 Immunization Record

Everyone who has received a COVID-19 vaccine in Prince Edward Island can access proof of their vaccine online or request a copy of their vaccine record through the COVID-19 Immunization Program. Immunization records include the date(s) of the COVID-19 vaccination(s) and the kind of vaccine(s) received.

Each person immunized against COVID-19 will have their own record of immunization, including children. You can have vaccines you received off Island added to your personal PEI immunization record.

How can I get a record of my COVID-19 immunization?  

If you provided your PEI Health Card number when you were vaccinated, you can access your vaccine record online anytime you need it at COVID-19 Proof of Vaccination.  

Please allow 24-48 hours after your vaccination for your information to be available to you online.

If you did not provide a PEI health card number at the time of vaccination or no longer have a PEI health card, contact the COVID-19 Immunization Program to request a copy of your record at vaxrecord@gov.pe.ca or 1-844-975-3303 (toll-free). Please include your full name and daytime phone number.

If you do not access your immunization online, you are advised to store your printed copy in a secure place for future use.

How do I add my off-Island vaccination information to my personal PEI Immunization Record?

Provide proof of your out-of-province COVID-19 vaccine to the PEI Chief Public Health Office as follows: 

Once your document is approved, it will be added to your personal PEI Immunization Record. 

What do I need to access my immunization record online?

Enter your PEI Health Card Number, you first and last names and date of birth to access your personal immunization record online. The information you enter must match Public Health records for information to display. 

What if there is an error on my immunization record?

To report an error or correction to your immunization information, please contact vaxrecord@gov.pe.ca or 1-844-975-3303 (toll free). 

Who can see my vaccine information? 

Immunization records are stored in an access-controlled registry in PEI. Under the PEI Immunization regulations, only health care providers, their administrative support staff, and public health officials can view your vaccine records.
