Submit Out-of-Province Vaccine Record for PEI Residents
This form is only used to update your PEI immunization record.
Island residents who received a COVID-19 vaccine outside PEI can have that vaccine added to their PEI Immunization record and receive a digital record of the vaccine for future use. Use the online form below to submit proof of out-of-province vaccine to the PEI Chief Public Health Office. Once your vaccine document is approved by CPHO, it will be added to your personal vaccine file.
What do I need to complete the online form?
You will be asked to provide:
- Clear identity and contact information, including your date of birth
- PEI Health Card Number
- Date and location COVID-19 vaccine was administered
- Accepted record of vaccine from the administering jurisdiction (upload field provided)
- Email address, if you choose to receive a digital copy of your vaccine record (CANImmunize will need your consent to send this to you)
How long will it take to process my request?
Allow up to 10 business days, or longer depending on volume of requests.
