Renew your Insulin Pump Program and/or Glucose Sensor Program benefits

You must reapply for the Insulin Pump Program and/or the Glucose Sensor Program each year between April 1 and June 30 to maintain your benefits. 

You can reapply in one of the following ways:

3. Obtain a paper form (for either program) at any Health PEI Diabetes Program location, complete and submit it by either mail or email (as a PDF only).

Am I eligible to renew my coverage?

You are eligible to renew your coverage for the Glucose Sensor Program if you:

  • have received benefits through the Glucose Sensor Program in the past 12 months,
  • are a PEI resident as defined by the Drug Cost Assistance Act
  • have a valid PEI Health Card,  
  • have filed your most recent income tax return in PEI.

You are eligible to renew your coverage of the Insulin Pump Program if you:

  • have received benefits through the Insulin Pump Program in the past 12 months,
  • are a permanent resident of PEI, 
  • have a valid PEI Health Card,  
  • have filed your most recent income tax return in PEI
  • continue to meet medical eligibility requirements for renewal.

When do I need to reapply?

Your benefits end on June 30 of each year. To keep your coverage, you must reapply annually between April 1 and June 30. We recommend you apply early, any time after April 1st (once your most recent income taxes are filed) to avoid interruption in benefits under the program as it may take up to 10 business days to process your renewal.

What do I need to reapply?

You will be asked to provide your:

  • PEI Health Card number
  • Social Insurance Number (SIN)
  • Private medical insurance coverage, if applicable
  • Annual household income on your tax return for the most recent year  

You will also need to provide this information for your spouse, if applicable.
