Primary Care Collaboration in Wellington

Collaboration between physicians, nurse practitioners, and nurses is driving quality care and provides more access to patients in need at the Evangeline Health Centre.

Recently, a unique collaborative clinic on PEI has opened to decrease wait times and improve access to care for Islanders.

The Evangeline Health Centre Same Day Clinic is available to patients whether or not they have a physician on PEI. It is open every Wednesday. Patients receive care from a team of primary care providers including a nurse practitioner, registered nurse, and a licensed practical nurse.

“The success of this clinic is significantly dependent on collaboration with all health care professional; the team at the clinic is what makes this story such a success,” said Darci Leggatt, Nurse Practitioner. “The clinic has provided me with the opportunity to provide care to islanders who may not have access to primary care (like a nurse practitioner or physician) for various reasons.”

A nurse practitioner is an autonomous health care provider with extensive medical training, advanced practice and can manage a broad catalogue of chronic and acute illnesses.

Nurse Practitioners can perform physical exams, order tests, diagnose and treat illnesses, write prescriptions, admit and discharge patients, and will also provide referrals if required.
